• Unencumbered, current Licensed Practical Nurse license in the state where the Home Care agency is located or compact state if applicable
• Graduate of a state approved LPN program
• Minimum one (1) year clinical experience as an LPN. Individuals with less than one full year’s experience will be considered and will receive mentorship and additional oversight for the first year.
• Experience in home health care is desirable.

• Under the supervision of a Registered Nurse, provide skilled nursing services as ordered on the plan of care and in accordance with policy/procedures and state practice acts
• Instruct the patient/caregiver in various aspects of patient care and disease management, and document patient/caregiver comprehension and compliance with teaching
• Observe patient response to care/teaching and recommend revisions to the plan of care to the Registered Nurse
• Provide and document skilled nursing care per the established plan of care, policies/procedures, and standards of care; perform other duties as assigned by the RN / Clinical Manager
