Overview

We are seeking a Part-time (3 full days per week), Advanced Practice Nurse Practitioner to join our growing House Calls PCP practice to see patients in Irving, HEB, Grapevine, North Arlington territory. Applicants must live near the locations posted and able to commute to the patients' homes. 1 year experience as a NP caring for geriatric patients, preferably in the home environment, is required.

Only qualified candidates will be considered.

• Perform physical exams and patient observations for geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
• Record patients’ medical histories and symptoms in EMR system.
• Create patient care plans and contribute to existing ones.
• Order, administer, and analyze diagnostic tests.
• Identify the need for and order medical equipment.
• Diagnose and treat complex health issues.
• Manage and assess effectiveness of medications and other treatments.
• Detect changes in patients’ health and modify treatment plans as needed.
• Consult with healthcare professionals and families.
• Train patients and their families to manage and prevent illness and injury.
• MIPS/Quality Measures.
• On-site collaborating/supervising physician.
• NO ON CALL.
• NO WEEKENDS.
• NO MAJOR HOLIDAYS.

• Ability to commute to patients' homes within Irving/HEB/Grapevine/North Arlington territory.
• On-site collaborating/supervising physician.
• NO ON CALL.
• NO WEEKENDS.
