About the Role:The Social Worker plays a critical role in supporting individuals, families, and communities to improve their well-being and address various social challenges. This position involves assessing client needs, developing care plans, and connecting clients with appropriate resources and services. The Social Worker advocates for vulnerable populations, ensuring their rights and dignity are upheld while promoting social justice. Collaboration with multidisciplinary teams and community organizations is essential to provide comprehensive support and interventions.

- Bachelor's degree in Social Work (BSW) from an accredited institution.
- Valid state licensure or certification as a Social Worker in the United States.
- Demonstrated experience working with diverse populations in social service settings.
- Strong knowledge of social work principles, ethics, and relevant laws and regulations.
- Excellent communication and interpersonal skills.
- Master's degree in Social Work (MSW) or related field.
- Experience with case management software and electronic health records.
- Specialized training or certification in areas such as mental health, substance abuse, or child welfare.
- Bilingual abilities or experience working with multicultural communities.
- Proven ability to work effectively in multidisciplinary teams.
- Conduct thorough assessments of clients' social, emotional, and environmental needs to develop individualized care plans.
- Provide counseling and support to individuals and families facing challenges such as mental health issues, substance abuse, poverty, or domestic violence.
- Coordinate with healthcare providers, government agencies, and community organizations to facilitate access to resources and services.
- Advocate on behalf of clients to ensure they receive fair treatment and necessary support within legal and social systems.
- Maintain detailed case records and documentation in compliance with organizational policies and regulatory requirements.
- Participate in multidisciplinary team meetings to discuss client progress and adjust care plans as needed.
- Engage in community outreach and education to raise awareness about social issues and available support services.
The Social Worker utilizes strong communication skills daily to build trust and rapport with clients from diverse backgrounds. Critical thinking and problem-solving abilities are essential for assessing complex situations and developing effective intervention plans.
