In deze rol ben je verantwoordelijk voor het regelen van verblijfsplekken voor ouderen in acute situaties en zorg je voor goede communicatie met huisartsen en verwijzers. Je hebt een relevante hbo-opleiding en ervaring in de ouderenzorg. Deze functie biedt een salaris conform cao VVT FWG 50 en uitstekende secundaire arbeidsvoorwaarden, met een contract van een jaar en de intentie tot verlenging.
