Een gemeente in Gelderland is op zoek naar een ervaren Wmo-consulent die verantwoordelijk is voor het coördineren en begeleiden van zorg- en ondersteuningsbehoeften. Je speelt een belangrijke rol in het verbeteren van de levenskwaliteit van cliënten door effectief samen te werken met diverse professionals. Met een hbo-opleiding in Social Work en minimaal 2 jaar ervaring, ben je in staat om empathisch en zelfstandig te werken.
