Arvada Care & Rehab in Arvada, CO is a post‑acute care facility offering long‑term care, short‑term care, and geriatric services. We provide a stable and supportive in‑house therapy team and excellent programs for our geriatric clientele on their journey to recovery.

• Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate care plan following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions for patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and modify services as needed; make referrals based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summaries of services per regulatory requirements.

Occupational Therapist license is required.

This position is open to OTs at all experience levels.
