Licensed Occupational Therapist (OT) – Care Coordination Program (PRN or Part-Time).

We’re looking for Occupational Therapists to support our innovative care coordination program. • Provide occupational therapy and wellness services for older adults.
• Conduct assessments, develop treatment plans, and implement therapeutic interventions as part of a multidisciplinary team.
• Support care coordination efforts by conducting regular wellness visits, communicating with patients’ providers and caregivers, and assisting patients with telehealth visits.
• Maintain professional standards in line with company policies and clinical initiatives.
• Contribute to program development, quality improvement, and problem-solving.
• Optimize each patient’s functional well-being and satisfaction.

Clinic hours typically run 8 a.m. to 5 p.m., Monday through Friday, with flexible scheduling to fit your lifestyle.
