Licensed Occupational Therapist (OT) – Care Coordination Program (PRN or Part-Time).
We’re looking for Occupational Therapists to support our innovative care coordination program. This opportunity empowers you to practice at the top of your license while expanding into care coordination duties designed to standardize hours and grow your career. Responsibilities- Provide occupational therapy and wellness services for older adults.
- Conduct assessments, develop treatment plans, and implement therapeutic interventions as part of a multidisciplinary team.
- Support care coordination efforts by conducting regular wellness visits, communicating with patients’ providers and caregivers, and assisting patients with telehealth visits.
- Maintain professional standards in line with company policies and clinical initiatives.
- Contribute to program development, quality improvement, and problem-solving.
- Optimize each patient’s functional well-being and satisfaction.
Clinic hours typically run 8 a.m. to 5 p.m., Monday through Friday, with flexible scheduling to fit your lifestyle.
