Great Lakes Management – LPN (Licensed Practical Nurse)
Join Our Team as an LPN (Licensed Practical Nurse)!
Are you passionate about seniors and assisted living? If so, we want you to be a part of our dynamic team at Great Lakes Management as an LPN! LPN (Licensed Practical Nurse) certification in the state of CO is required.
Base pay range: $33.00/hr – $35.00/hr

- Implement service agreements with all new residents. Review and revise service plan as needed with a minimum of one review per year.
- Administer medications accurately and at appropriate times according to physician and resident service plan.
- Document medications administered and those not given according to regulations and policies.
- Monitor health, safety and well‑being of the residents.
- Document changes in resident’s functional status, personal needs, appointments, vitals, etc.
- Complete incident reports and documentation on resident’s record.
- Respond to resident emergencies; provide first‑aid assistance and arrange for appropriate medical attention/follow‑up.
- Report concerns about residents to the Resident Care Coordinator/Director of Nursing and/or Executive Director.
- Assist residents with activities of daily living, including bathing, dressing, eating, personal hygiene and mobility.
- Assist residents with oxygen and other medical problems outlined in the assistance plan.
- Assist with resident laundry, daily bed making, and serving meals, as requested.
- Identify supplies needed for tasks and report needs to DON.
- Encourage socialization among residents and promote activities.
- Chaperone activities outside the facility when requested.
- Maintain a safe, home‑like, and pleasant environment for residents.
- Practice safety around equipment, chemicals, tools, and utensils.
- Attend regularly scheduled staff meetings and in‑services.
- Report on‑job injuries to the supervisor before the end of the work shift on the day the injury occurs.
- Maintain confidentiality of verbal and written information pertaining to residents, facility operations and personnel.
- Monitor and maintain resident charts and clinical progress notes.
- Communicate to physicians, staff and family changes in resident’s needs or conditions.
- Facilitate coordination of resident services with other service providers.
- Oversee medications and renewals; follow up with physicians about medications and/or medical conditions.
- Ensure all physician orders are signed and implemented within 24 hours.
- Ensure medication administration procedures are followed properly and discuss needed corrections, errors, etc., with staff.
- Communicate in resident’s log any information that staff need to be aware of.
- Work as a team player with all members of the resident’s care team.
- Share resident and family concerns with pertinent staff if appropriate.
- Attend monthly staff meetings.
- Work with manager to assure emergency procedures are in place.
- Be flexible and willing to expand and adapt to changes in the assisted‑living industry.
- Promote a positive, team approach and a healthy work environment.
- Ensure compliance with regulations and be available for federal and state licensure surveys.
- Maintain a current state license as a Licensed Practical Nurse per state regulations.
- At least one year of experience in home health, assisted living or long‑term care is preferred.
- Good organizational skills with the ability to self‑start and follow through with initiatives with minimal supervision.
- Exhibit a caring heart with a passion to care for the elderly.
- Exceptional verbal and written communication skills.
- Experience using a resident record computerized system.
- Experience working with seniors.
