Registered Practical Nurse – Transitional Care Program – Talent Pool

Position Summary As a member of a multi‑disciplinary team, the Registered Practical Nurse practices in accordance with relevant legislation and UHN professional standards to provide compassionate, collaborative and competent care. Responsibilities include managing nursing care for patients and families within the RPN scope of practice, participating in inter‑professional team planning, providing mentorship to students and new staff, and supporting patient safety initiatives.
Duties
- Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of interdisciplinary practice.
- Use current theoretical and research knowledge and the input of team members to plan and implement care for patients and families.
- Participate in inter‑professional team care planning forums, rounds, and family conferences.
- Provide mentorship to RPN students and new staff on the unit.
- Participate in professional, program and hospital‑wide initiatives, including professional development.
- Ensure compliance with patient safety responsibilities and hospital plans, policies, and procedures.
- Maintain valid registration with the College of Nurses of Ontario and a Diploma in Practical Nursing.
- Apply effective problem recognition, problem solving and decision making skills.
- Demonstrate commitment to continuing professional development, team success, and resilience.
- Show computer literacy in word processing and Electronic Patient Record systems.
- Maintain satisfactory attendance and punctuality.
Qualifications
- Valid registration with the College of Nurses of Ontario.
- Diploma in Practical Nursing.
- Recent related clinical experience preferred.
- Effective problem recognition, problem solving, and decision making skills.
- Demonstrated commitment to continuing professional development.
- Demonstrated commitment to team success.
- Resilience and effective coping strategies to manage change.
- Demonstrated computer literacy in word processing and Electronic Patient Record.
- Working knowledge of the College of Nurses of Ontario standards of practice.
- Evidence of satisfactory attendance and punctuality.
