MDS Coordinator – Part Time Company

- Assist with the development of comprehensive care plans for residents in coordination with the MDS to accurately address resident needs.
- Coordinate, manage, and monitor the written plan of care for each resident, identifying needs and goals.
- Assist nursing management with the coordination, management, and review of nurses’ notes to determine if the care plan is being followed.
- Monitor resident status changes to ensure appropriate and timely nursing or clinical involvement.
- Ensure MDS and support documentation accurately represent the resident and meet regulatory and auditor requirements.
- Perform regular audits of documentation to assure accuracy.
- Assist nursing management with the discharge process.
- Perform administrative requirements, such as completing necessary forms and reports.
- Maintain infection control and standard precaution practices when providing care, following established safety precautions.
- Maintain strict confidentiality regarding sensitive health information of residents.
- Report all hazardous conditions, damaged equipment, and supply issues to appropriate persons.
- Maintain the comfort, privacy, and dignity of residents, interacting with them with warmth and respect to promote a caring environment.
- Answer and respond to call lights promptly and courteously in resident care areas.
- Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in‑services as directed.
Education and Experience
- Current State License as a Nurse
- CPR Certified
Education and Experience
- One year experience as a nurse in a long‑term care setting.
- Knowledge and training in all aspects of the MDS process.
