Essential Functions
  • Keep abreast of current federal and state regulations, as well as professional standards.
  • Assist with the development of comprehensive care plans for residents in coordination with the MDS to accurately address resident needs.
  • Coordinate, manage, and monitor the written plan of care for each resident, identifying needs and goals.
  • Assist nursing management with the coordination, management, and review of nurses’ notes to determine if the care plan is being followed.
  • Monitor resident status changes to ensure appropriate and timely nursing or clinical involvement.
  • Ensure MDS and support documentation accurately represent the resident and meet regulatory and auditor requirements.
  • Perform regular audits of documentation to assure accuracy.
  • Assist nursing management with the discharge process.
  • Perform administrative requirements, such as completing necessary forms and reports.
  • Maintain infection control and standard precaution practices when providing care, following established safety precautions.
  • Maintain strict confidentiality regarding sensitive health information of residents.
  • Report all hazardous conditions, damaged equipment, and supply issues to appropriate persons.
  • Maintain the comfort, privacy, and dignity of residents, interacting with them with warmth and respect to promote a caring environment.
  • Answer and respond to call lights promptly and courteously in resident care areas.
  • Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in‑services as directed.
Required

Education and Experience
  • Current State License as a Nurse
  • CPR Certified
Preferred

Education and Experience
  • One year experience as a nurse in a long‑term care setting.
Additional Eligibility Qualifications
  • Knowledge and training in all aspects of the MDS process.
