You will provide high‑quality, primary‑care services to residents of SNFs, assisted living, and independent living communities. The role emphasizes preventive care, chronic disease management, and coordinated care with multidisciplinary teams.

• Conduct comprehensive assessments, develop treatment plans, and manage ongoing patient care for residents.
• Perform routine rounds, monitor resident conditions, and adjust treatment plans as needed.
• Collaborate with facility staff, families, and other healthcare providers to ensure coordinated, resident‑centered care.
• Educate residents, families, and staff on disease prevention, wellness, and chronic illness management.
• Maintain accurate, timely documentation in the electronic medical record (EMR) and adhere to regulatory and institutional standards.
• Participate in quality improvement initiatives to reduce readmissions and improve patient satisfaction.

• Valid nurse practitioner or physician assistant license in the state of operation and current board certification in your field.
• Master’s degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem‑solving skills.
• Excellent written and oral communication skills.
• Organization, punctuality, and ability to function within a team.

This role requires travel to and from assigned facilities. Depending on the travel requirement, a valid driver’s license, current auto insurance, and a clean driving record may be required. Periodic motor vehicle record (MVR) checks may be conducted as a condition of continued employment.
