SummaryYou will provide high‑quality, primary‑care services to residents of SNFs, assisted living, and independent living communities. The role emphasizes preventive care, chronic disease management, and coordinated care with multidisciplinary teams.
Essential Duties & Responsibilities- Conduct comprehensive assessments, develop treatment plans, and manage ongoing patient care for residents.
- Perform routine rounds, monitor resident conditions, and adjust treatment plans as needed.
- Collaborate with facility staff, families, and other healthcare providers to ensure coordinated, resident‑centered care.
- Educate residents, families, and staff on disease prevention, wellness, and chronic illness management.
- Maintain accurate, timely documentation in the electronic medical record (EMR) and adhere to regulatory and institutional standards.
- Participate in quality improvement initiatives to reduce readmissions and improve patient satisfaction.

- Valid nurse practitioner or physician assistant license in the state of operation and current board certification in your field.
- Master’s degree in Nursing or Physician Assistant Studies.

This role requires travel to and from assigned facilities. Depending on the travel requirement, a valid driver’s license, current auto insurance, and a clean driving record may be required. Periodic motor vehicle record (MVR) checks may be conducted as a condition of continued employment.
