The medication history pharmacy technician performs medication histories for new hospital admissions.

Duties include:

• Collecting complete medication information—drug name, dose, route, frequency, indication, and date/time last dose—using patient/family interview, medication lists, outside facility records, outpatient pharmacy records, and physician office records.
• Verifying and updating patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record.
• Documenting medication histories in the electronic health record, sending them to the pharmacist for cosignature, and recording them in CPW/Vigi Lanz.
• Assisting the medication reconciliation pharmacist by ensuring patients identified for discharge counseling and medication reconciliation have a pharmacy‑services‑completed medication history.
• Preparing education materials for the medication reconciliation pharmacist as needed.
• Working flexible shifts—weekend, evening, holiday, on‑call, and additional hours—as required by the facility/department.

• Required education:
  High school graduate or GED.
• Required license:
  Certified Pharmacy Technician (CPhT).
• Required experience:
  
  >2 years of previous retail pharmacy experience.
• Preferred experience:
  
  >1 year of previous inpatient pharmacy experience.
