Position: Pharmacist (H)-Ambulatory

JOB SUMMARY

  • Responsible for dispensing and controlling both prescription and non‑prescription medicine, coordinating drug distribution and clinical pharmacy activities. Is responsible for safe and efficient methods for coordination of established pharmacy policy concerning drug and patient care. Provide clinical drug information and advice about health issues, symptoms and medications in response to customer inquiries. Training and managing staff.
DUTIES AND ESSENTIAL FUNCTIONS
  • Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
  • Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
  • Controls medications by monitoring drug therapies; advising interventions.
  • Completes pharmacy operational requirements by organizing and directing technicians' work flow; verifying their preparation and labeling of pharmaceuticals; verifying order entries, charges, and inspections.
  • Provides pharmacological information by answering questions and requests of health care professionals; counseling patients on drug therapies as well as perform these tasks in the Ambulatory Setting.
  • Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by monitoring nursing unit inspections; maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory; supervising the work results of support personnel; maintaining current registration; studying existing and new legislation; anticipating legislation;

    advising management on needed actions.
  • Administer approved CDC immunizations as requested
    • Clinical Oncology Pharmacist / Infusion Center Pharmacist
    • Leads and manages oncolytic medication initiation and/or infusion center workflow including oversight of technicians, interns, students, residents and volunteers ensuring accuracy and adherence in accordance with departmental procedures and Federal and State regulations, including USP 797/800.
    • Participates in departmental meetings and serves on committees within the health system as appropriate.
    • Presents as oncolytic and/or medication therapy resource for activities such as drug information, training of staff, transitions of care and medication therapy services by maintaining updated knowledge and skills, as well as educating nursing staff and providers when applicable.
    • Evaluates medication orders, including chemotherapy and/or oncology orders, for appropriateness with regard to patient condition, organ function and concomitant therapies to promote optimal patient outcomes.
    • Recognizes opportunities and offers solutions to maximize efficiency and promote safety regarding oral medication use, including but not limited to prior authorizations, appeals processes, financial assistance applications and manufacturer free drug program applications.
    • Provides clinical review for oncology treatment plan builds and modifications (and other supportive therapy plans) to determine accuracy and standard of care relevance, along with performing chemotherapy patient education for newly applied treatment plans.
    • Serves as pharmacy asset for all hematology/oncology providers, nurses, and patients, as well as all infusion clinics if applicable by being the medication expert within multidisciplinary interactions, rounds and patient visits.
    • Oversees patient therapy navigation by working in conjunction with retail/specialty pharmacies and insurance companies regarding oral oncolytic refills, drug interactions, dose changes, adherence monitoring, and side effect management. (if applicable)
    • Responsible for appropriateness and accuracy of all compounding to ensure safe medication preparation and delivery.
    • Maintains updated knowledge and skills, as well as provides education to nursing staff, providers and other team members as appropriate.
    • Works closely with clinic staff, nurses, mid‑level providers, and physicians to meet the operational, clinical, financial, and research needs of the cancer center.
