Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Ashland, KY. This travel role focuses on providing home health physical therapy services, ensuring patients achieve maximum independence and function. - Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for patient independence
- Regularly reassess treatment effectiveness and patient progress
- Communicate patient needs and progress to treatment team and family
- Consult with medical providers on patient evaluation, treatment, and discharge
- Assist in securing and incorporating special adaptive equipment
- Maintain and submit proper documentation as per regulations
- Supervise Physical Therapist Assistants and Aides in patient care
- Attend and participate in staff development programs
- KY Physical Therapist Licensure or Compact Licensure
- 1 year home health physical therapy experience
- Previous oasis experience
- Experience with Home Care Home Base
