• Deliver patient-centered care that brings warmth and dignity to every interaction
• Administer medications and treatments safely and accurately
• Monitor residents  health and document changes with precision
• Collaborate with a multidisciplinary team to create and update individualized care plans
• Maintain a clean, safe environment that promotes comfort and wellness
• Communicate with residents, families, and team members to support whole-person care

• Active LPN license in the State of Ohio
• Current CPR certification
• Experience in skilled nursing, assisted living, or long-term care preferred
• Strong clinical skills and attention to detail
• Compassionate mindset with excellent communication and teamwork abilities
