Care Coordinator – Ravenwood Health

Base pay range: $22.00/hr – $24.00/hr
Full‑time, high‑fidelity wraparound, community‑based services to youth 0‑20 with behavioral health challenges located in Ashtabula County and neighboring counties.

- Regularly engage with youth and families on a culturally competent, community‑based level.
- Assess level of care using the CANS assessment.
- Connect youth and families to resources, natural supports, and collaborate with wraparound teams.
- Organize wraparound team meetings and maintain documentation of these meetings.
- Link youth and families to specialized clinical resources to meet treatment needs.
- Provide assistance and support in crisis situations; respond effectively and mobilize access to resources.
- Work closely with youth and families to develop care plans incorporating their voice and choice; routinely monitor and adjust plans.
- Collect outcome information by administering and collecting necessary forms to client and family.
- Participate in regular training regarding health equity, health disparities, and cultural competence.
- Minimum 3 years’ experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services or behavioral health care field, providing community‑based services to children and youth.
- Valid driver’s license.
- Background check pass.
