Zorggroep Drenthe zoekt een zorgadviseur die cliënten en hun naasten ondersteunt bij beslissingen rondom wonen, zorg en ondersteuning. Je fungeert als verbinding tussen cliënt, verwijzer en zorgorganisatie en zorgt dat de juiste zorg op de juiste plek terechtkomt.
Je werkt grotendeels zelfstandig maar werkt intensief samen met collega zorgadviseurs en andere professionals. Een relevante HBO-opleiding en ervaring in de VVT-sector zijn vereist.
