Zorgplein Noord in Assen zoekt een verpleegkundige voor het outreachende team. In deze rol bied je intensieve zorg aan cliënten met verslavingsproblemen en werk je nauw samen met collega's vanuit een multidisciplinair team.
Je bent verantwoordelijk voor de methadonbehandeling en het opstellen van zorgplannen, waarbij je cliënten bezoekt in hun eigen leefomgeving. Een afgeronde HBO-opleiding Verpleegkunde en affiniteit met verslavingszorg zijn vereist. De functie biedt ruimte voor ontwikkeling en een aantrekkelijk dienstverband.
