Voor onze huisartsenpraktijk zijn wij op zoek naar een enthousiaste verpleegkundige POH-S voor 16 uur per week.
Werk je graag zelfstandig binnen een multidisciplinair team? Dan maken we graag kennis met je.
Wij zijn op zoek naar een collega die:
- Beschikt over een diploma voor verpleegkundige
- De opleiding POH-s heeft afgerond
- Ervaring heeft met chronische zorg en patiënten begeleiding
- Zelfstandig kan werken en communicatieve vaardigheden heeft
- Patiënt gericht, zorgvuldig en collegiaal is
- Affiniteit heeft met preventieve zorg en leefstijlbegeleiding
- Een prettige werksfeer met korte lijnen
- Werken in een betrokken en professioneel team
- Salaris en arbeidsvoorwaarden conform de CAO huisartsenzorg
- Mogelijkheden voor scholing en professionele ontwikkeling
- Ruimte voor eigen initiatief en inbreng
