FACT Jeugdteam Assen zoekt een Ambulant hulpverlener die verbindend en mensgericht werkt. Je werkt met jongeren tot 23 jaar en gezinnen met complexe problematiek vanuit een multidisciplinair FACT-team in Assen.
Je bent geregistreerd jeugd- en gezinsprofessional met ervaring in outreach en ambulant werk. Je levert flexibele, creatieve begeleiding en onderhoudt contact met ketenpartners zoals scholen en jeugdbescherming.
