Werken aan het Noorden zoekt een Casusbegeleider voor 16 uur per week, van 17-08-2026 tot 31-12-2026, ten bate van Moeders van Midden-Groningen. Je ondersteunt (aanstaande) moeders en gezinnen in een kwetsbare situatie gedurende de eerste 1.000 dagen van het kind, en levert praktische en integrale zorg op gebieden zoals opvoeding, financiën en wonen.
Je werkt nauw samen met netwerkpartners, draagt bij aan vroegsignalering en begeleidt studenten in het team.
