Zorgplein Noord zoekt een bevlogen ambulant hulpverlener voor het FACT Jeugdteam in Assen. Je werkt nauw samen met collega’s van verschillende jeugdorganisaties in Drenthe en levert maatwerk voor jongeren en hun gezin.
Je neemt deel aan zorgnetwerken, bent ambassadeur van VNN en draagt bij aan het terugdringen van problematisch middelengebruik. Je werkt in een dynamisch team waar humor en betrokkenheid centraal staan.
