Addiction treatment Northern Netherlands in Assen zoekt een ambulante hulpverlener in de outreachende verslavingszorg. Je bezoekt cliënten thuis, bouwt aan vertrouwen en coördineert behandelingen met familie en betrokkenen.
Je werkt binnen een multidisciplinair team, hebt een caseload na inwerkperiode en zet herstelondersteunende zorg in. Arbeidsvoorwaarden volgens CAO GGZ en een jaarlijks balansbudget zijn onderdeel van het pakket.
