The Veterans Affairs Portland Healthcare System, North Coast Healthcare Clinic in Astoria, OR, is seeking full-time board-certified/board-eligible Family Practice or Internal Medicine Physicians to join our Primary Care Health Team. The primary care provider will serve as a highly skilled, independent care provider in managing patients within an interdisciplinary care team, utilizing state-of-the-art medicine and advanced medical technologies.
The Health Care Clinic operates under the PACT Model (Patient Aligned Care Team). The team, consisting of a PCP, RN, LPN, and a Medical Support Assistant, cares for a panel of approximately 1200 patients. The PACT model provides comprehensive care for Veterans, including diagnoses, care, and treatment of both acute and chronic conditions. Common diagnoses include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain.
The position is Monday through Friday, with no weekends or holidays.
Although Astoria is remote from the Portland VA, there are multiple connections to the main facility. The Portland VA has a Community-Based Out-Patient Clinic in Astoria, multiple primary care providers, and sub-specialty support. Veterans can receive consultations via electronic consults or Tele-health and may receive sub-specialty care through co-management with the private sector or appointments at the Portland VA.
Candidates must be board certified or eligible in Internal Medicine, Family Medicine, or Geriatrics and have significant experience in the primary care of complex patients through interdisciplinary team management. Teaching experience is desirable.

Expected salary range for well-qualified physicians is competitive for the academic rank, with consideration given to candidates' qualifications, credentials, and experience. It may also include up to $15,000 in Annual Performance Bonus. Selected candidates may be eligible to apply for the Education Debt Reduction Program (EDRP) and, if approved, receive up to $200k in school loan repayment.
