• Location: Astoria, OR
• Employment Type: Full‑time, 1.0 FTE (40 hours per week)
• Salary: $132,000 – $144,000 annually plus performance incentives
• Additional Compensation: $15,000 hiring bonus, $20,000 relocation allowance, $25,000 geographical pay, $1,500 monthly housing/auto stipend, $50,000 student loan repayment assistance
• Benefits: Medical, dental and vision at no cost; paid time off, eight paid holidays, $3,100 CME allowance, 401(a) profit‑sharing plan, 403(b) retirement plan, life insurance, employee assistance program, 4‑week Medical Spanish immersion program in Guatemala

• Provide primary and preventive medical care to patients, managing a patient panel in a community health center setting.
• Deliver evidence‑based care consistent with medical home methodology and address health needs of underserved populations.
• Collaborate with the multidisciplinary team to coordinate comprehensive care and referrals.
• Maintain accurate clinical documentation in the EPIC electronic medical record system.
• Participate in quality improvement initiatives, including patient satisfaction and productivity metrics.
• Conduct medical Spanish immersion and participate in language translation support for bilingual patients.

• Master’s or Doctoral degree in Nursing (Nurse Practitioner) or Master’s degree in Physician Assistant; or Bachelor’s degree in Health Science with a two‑year Physician Assistant training program.
• Current, valid state licensure as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant). Additional certifications (e.g., BLS, ACLS) preferred.
• Experience in community health, family medicine, or primary care is desired.
• Commitment to serving diverse, underserved populations and inclusion.
• Excellent communication and interpersonal skills, and bilingual ability strongly preferred.
