What You’ll Do
- Assess patients’ current situation and identify services needed to ensure basic needs and concerns are addressed.
- Administer the “Social Determinants of Health” tool to facilitate disclosure of additional needs or concerns such as mental health, nutrition, physical activity, and/or finances.
- Make referrals to appropriate agencies and service providers based on identified needs and provide information and assistance to clients for self‑referral.
- Utilize local community resources and services to provide recommendations and referrals for clients, influencing their responsiveness to accessing recommended resources.
- Identify and provide outreach to individuals and families in need of healthcare coverage or home services, including special populations such as migrants and the homeless.
- Provide application assistance for clients for healthcare coverage as needed.
- Collaborate with all staff members to provide comprehensive, individualized services.
- Provide education to individuals and families on effective use of the local healthcare system and available agencies.
- Participate in community outreach functions as requested.
- Complete necessary documentation, including the Monthly Tracker.
- Perform other duties as assigned.
Qualifications
- High School Diploma or GED.
- One year experience as a Lay Leader, Promotor, Community Health Worker, or similar role. A degree or Community Health Worker certification may substitute for experience.
- Valid driver’s license and proof of automobile liability insurance coverage.
- Bilingual (English/Spanish) preferred at level 10.
- Effective verbal, written, and listening communication skills.
- Strong organizational skills.
- Ability to analyze and address patient concerns and identify priorities for patient care.
- Ability to prioritize work and handle multiple tasks simultaneously with frequent interruptions.
- Knowledge of local social and health agencies and services offered.
- Knowledge of medical terminology.
- Knowledge or membership of the community being served.
- Basic knowledge of health promotion, disease prevention, and preventive care principles.
- Basic proficiency with computer programs including Word and Excel.
