Position: Nurse (PACT - Outpatient)

Summary

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides gender‑specific direct Veteran care and coordination—as it pertains—to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.

Qualifications
  • English Language Proficiency
  • In accordance with 38 U.S.C. 7403(f) – no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
  • Graduate of a professional nursing school approved by an accrediting agency (ACEN or CCNE) at the time of program completion.
  • OR individuals attending a master’s level bridge program in nursing who completed coursework equivalent to a bachelor’s degree in Nursing may become registered as a nurse with a state licensing board prior to completion of the bridge program.
  • Upon achievement of a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
  • OR graduates of foreign nursing schools – possession of current, full, active, and unrestricted registration meets the requirement for appointment as a registered nurse.
  • Credit for foreign nursing education higher than an associate degree requires a formal equivalency validation from an accepted agency such as ICD.
  • Current full, active, and unrestricted registration as a graduated professional nurse in a U.S. State, Territory, Commonwealth (e.g., Puerto Rico), or the District of Columbia.
  • Graduate Nurse Technician (GNT) Exception – candidates who meet educational requirements but lack licensure can be appointed at the entry step of the grade and later undergo a temporary appointment while actively pursuing licensure.

  • Physical Requirements:

    Moderate lifting (15‑44 lbs), moderate carrying (15‑44 lbs), straight pulling (0.5 hrs), pushing (0.5 hrs), reaching above shoulder, use of fingers, both hands, walking (4 hrs), standing (4 hrs), kneeling (0.5 hrs), mental and muscular coordination, depth perception, color discrimination, hearing (aid permitted), mental and emotional stability.
  • Environmental Factors:
    Working closely with others.
Duties
  • Provides health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
  • Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
  • Demonstrates performance and leadership to improve care quality in the Primary Care Department and facility.
  • Provides peers with formal or informal constructive feedback for improvement.
  • Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care.
  • Fosters a safe and supportive environment conducive to professional development of health care professionals.
  • Evaluates outpatient outcomes of evidence‑based decisions and practice changes for primary care individuals, groups, and populations.
  • Responsible for documented PACT outcomes at the program or service level.
  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Assesses, plans, implements, and evaluates care based on age‑specific components.
  • Assumes responsibility for coordination of care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
