A major rural healthcare provider is seeking a Health Navigator to develop relationships with patients and assist with care coordination. This role involves entering patient data, scheduling appointments, educating patients on healthcare options, and coordinating follow-up care. Ideal candidates should have a service mentality, problem-solving skills, and experience using electronic health records. A GED or High School Diploma is required.

The position is full-time with hours Monday to Friday, 8 AM to 5 PM.
