JOB PURPOSE:
Performs duties of distribution, documentation and compounding related to drugs, solutions and equipment to meet the needs of patients. Responsible for the reviewing, compounding, dispensing and monitoring of medication therapy for home infusion and specialty pharmacy patients, including dosing variations, nutritional needs and disease states associated with various age groups. Supervises pharmacy technician activities, including monitoring of all orders filled by auxiliary personnel.
Assumes responsibility for competency and self‑development in professional pharmacy practice and maintains a working knowledge of the current USP 797/800 rules affecting the home infusion pharmacy. Performs patient‑focused care clinical activities through regular phone contact with patients to ensure medication adherence, optimal medication therapy outcomes, and compliance with The Joint Commission Standards for Homecare. Communicates with other medical personnel involved in the care of the home patient.
Works with billing and intake staff to ensure quality care coordination and proper billing guidelines are followed.
Minimum
Education Required:
Graduate of an ACPE accredited school of pharmacy with a Bachelor’s Degree or higher in Pharmacy.

Minimum Licensure/Certification Required By Law: Licensed as a registered pharmacist in the state of Georgia.
Additional
Preferred Qualifications:
One year experience in acute care or Home Infusion preferred.
