- American Heart Association BLS certification required for all direct patient care personnel
- American Heart Association ACLS certification required for RNs in CCU, ED, PACU, MICU, SICU (Step Down unit included in critical care areas)
- Fit Test required
- Trinity 10 Panel Drug Screen required within 30 days of start date
- Trinity Code of Conduct Form required
- Trinity I9 form required
- Confidentiality and Security Agreement required
- Integrity and Audit Services Form required
- Trinity Skills Checklist required for clinical contractors
- COVID-19 Vaccine proof or exemption required
- MMR vaccination proof or IgG titer required
- Hepatitis B vaccination series required
- Influenza Vaccine proof or exemption required
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required
- Varicella vaccination proof or IgG titer required
- Physical/Health Statement within last year required
- Criminal Background Check within 30 days required
