• Coordinate with social service providers to ensure comprehensive family care
• Participate in team meetings and case reviews
• Assess family needs and develop plans to address them
• Provide case management services such as supervised visits, parent aide, behavioral aide, drug screens, crisis intervention, and advocacy
• Conduct regular home visits to monitor progress and address issues
• Offer parenting education and support to improve family dynamics
• Collaborate with professionals like social workers, therapists, and teachers
• Maintain detailed case notes and records
• Provide transportation services

• Bachelor’s Degree in human services disciplines such as Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc., or a High School Diploma with 5 years of relevant experience
