The Youth Tracker is responsible for completing assigned daily contact with youth, as well as meeting the conditions of the youth’s service plan for their assigned caseload. The Youth Tracker must maintain a flexible work schedule that may include working hours outside of the regular schedule, including evenings, weekends, and holidays.
Essential Functions:- Conduct face-to-face daily contacts at various locations with assigned youth.
- Manage behaviors through programmatic and individual structure.
- Develop and maintain helping relationships emphasizing youth accountability.
- Complete required documentation, including discharge information, accurately and timely.
- Enter data promptly into the Department of Juvenile Justice’s Juvenile Tracking System (JTS) and Southwest Key Programs database, Efforts to Outcome (ETO).
- Review case files to familiarize with youth history and keep information updated.
- Notify the Program Director or designee immediately in case of youth crisis, suspected illegal activity, or any other incident.
- Maintain regular and punctual attendance.
- Assist in coordinating youth activities and groups as needed.
- Provide staff relief and be available for on-call assistance.
- Advocate for youth in health, education, vocation, legal, and family areas.
- Assist with training new staff as directed.
- Provide crisis intervention and administer first aid when necessary.
- React productively to change and handle other essential tasks as assigned.
- High School Diploma or GED and at least 3 months of experience working with youth and families or a related field.
- Maintain a clean driving record, active driver’s license, registration, and vehicle insurance, to be provided to the Program Director before expiration.
- Reliable transportation, registration, and maintained state-mandated car insurance; responsible for renewal documents.
- Clear results from a criminal background check by the Georgia Department of Juvenile Justice.
- Clear drug screen results from the organization’s designated medical facility.
- Good verbal and written communication skills.
- Ability to perform goal-oriented casework.
- Ability to manage conflict and perform well in crisis situations.
- Ability to confront clients with problematic behaviors and set limits.
Must obtain First Aid Certification (CPR). Ability to verify client whereabouts by driving, sitting, standing, and walking for extended periods. Daily use of office equipment. Bending, stooping, climbing stairs, and lifting up to 15 lbs. are required.
This program requires daily contact at various client locations, with shifts subject to change. Noise levels vary from mild to moderate. Extended periods in an automobile may be necessary. Exposure to weather conditions. Travel is approximately 85% to various GA counties (up to 1.5 hours one way) and 15% in the office. Staff must use their own vehicle for tracking; travel reimbursement is provided at IRS mileage rates.
