Hospice Registered Nurse

As a Case Manager (RN), you will play a crucial role in providing high-quality care to our patients while promoting their independence, dignity, and quality of life.

We are seeking a Registered Nurse (RN) to join our team as a Case Manager at Buckeye Hospice and Palliative Care Services in Zanesville, Ohio. The ideal candidate will have a passion for delivering patient-centered care and be committed to making a difference in the lives of those we serve.

• Provide initial and comprehensive assessments of patient needs, developing personalized care plans to address their health and social needs.
• Implement and revise care plans as needed, ensuring that patients receive the best possible care and services.
• Communicate effectively with patients, families, and healthcare teams to coordinate care delivery and ensure seamless transitions.
• Supervise and instruct direct care staff members and non-professional hospice aide staff to deliver quality care.
• Participate in on-call rotation, providing 24/7 coverage for patients and families in need.

• RN License is REQUIRED.
• Valid Driver's License and reliable transportation that is insured in accordance with Ohio state requirements.
• Minimum of 1-4 years of experience as a Registered Nurse working with hospice services or home health agency.
• On-Call Rotation.

• Multiple locations in Zanesville and surrounding areas.
• Long-term care settings, including nursing facilities and home settings.
• Monday to Friday, with occasional on-call and weekend availability.
