Description

PT-FT

• Provides for the individual rights of patients receiving services of the medical center.
• Directs/provides for the appropriate assessment and re-assessment of patients, including collection of historical data, observation of the patient's physical, psychological, social, and spiritual needs as they relate to the patient's age.
• Provides appropriate therapy based on the individual patient's assessed needs.
• Provides healthcare education to patients and/or families pertinent to assessed educational needs.
• Provides continuity of care by enlisting the services of other healthcare team members to plan a timely and trouble-free discharge to the level of care appropriate to the patient's condition.
• Manages the patient's care to ensure patient safety and to minimize risk to the patient, family, and the organization.
• Manages the patient's environment of care and care unit to ensure environmental safety.
• Manages time and supplies efficiently in the care of the patient to ensure cost-effective and efficient use of resources.
• Communicates pertinent information to appropriate individuals involved in care of the patient.
• Accurately documents the activities of rehab in the patient record in a complete and timely manner.
• Possess necessary knowledge use of patient/organizational information systems.
• Demonstrates safety in infection control and Haz Mat practices.
