- Provide 1:1 care for clients following their plan of care while maintaining dignity and independence.
- Administer medications, feedings, oxygen, and ostomy care as needed.
- Accurately document nursing actions, communicate with family, case manager, and physicians, and update progress toward established goals.
- Coordinate or order supplies, medications, and feedings to ensure adequate availability.
- Ensure all patient needs are met in accordance with physician instructions.
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
- Valid driver’s license, safe driving record, and proof of car insurance.
- CPR professional certification.
- 1+ years of nursing experience preferred.
- Home Health care experience preferred.
