Part Time and Full time opportunities available to deliver personalized, in-home healthcare services to adult patients with unique and evolving needs.

• Deliver high-quality, individualized nursing care to patients in their homes as prescribed by the physician.
• Collaborate with an interdisciplinary care team to develop, implement, and adjust the patient’s plan of care.
• Monitor and evaluate patient progress, reporting findings to the physician, case manager, and family as appropriate.
• Administer treatments and manage nursing interventions based on clinical assessments.
• Participate in on-call rotation (approximately every 3–4 weeks) and perform PRN visits as needed.
• Complete documentation using Kinnser (Well Sky) system.
• Other duties as assigned.

• Current RN license in the state of Texas
• Minimum 1 year of RN experience (Home Health preferred)
• Proficiency in English (spoken and written)
• Valid Texas driver’s license and current auto liability insurance
• Current CPR/BLS certification
• Familiarity with Kinnser/Well Sky is a plus
