We’re looking for a compassionate and skilled After hours Registered Nurse (RN) to join our Advanced Illness Management (AIM) team. In this vital palliative care role, you’ll provide after‑hours clinical support to patients and families facing serious illness—through in‑person visits, phone triage, and virtual interventions.

Work schedule is seven days on, seven days off. 5 pm–8 am during the week, and 5 pm Friday through 8 am Monday.

Key Responsibilities
  • Triage and respond to urgent calls from palliative care patients and families after hours
  • Conduct initial nursing evaluations to determine eligibility for palliative care services
  • Provide clinical support via phone, video, or in‑person visits as needed
  • Collaborate with physicians, nurse practitioners, and the interdisciplinary team (IDT)
  • Assess and manage patients’ physical, emotional, psychosocial, and spiritual needs
  • Document care accurately and timely in the electronic medical record (EMR)
  • Communicate changes in condition to the appropriate care team, including PCPs and supervisors
  • Assist with medication, DME, and supply needs in accordance with care plans
  • Ensure continuity of care and contribute to advanced illness care planning
  • Audit patient charts as needed
Education
  • Associate or bachelor’s degree in Nursing
About You
  • 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide quality patient care
  • Experience in palliative care, hospice, home health, oncology, or geriatric care preferred
  • Prior experience in triage nursing, emergency care, or crisis response is a plus
  • Additional preferred experience: hospital Staff RN, Med‑Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing
Licenses, Certifications And/or Registration
  • Active and unrestricted RN license in the state with no board actions
  • CPR Certification
Equipment/Tools/Work‑Aids
  • Familiarity with electronic medical records (EMR) and Microsoft Office
Specialized Knowledge And Skills
  • Competent to perform after hours in‑person visits
  • Knowledge of palliative care principles, symptom management, and patient advocacy
  • Strong assessment and critical thinking skills
  • Strong, compassionate phone communication
  • Ability to work independently with sound clinical judgment
  • Independent decision‑making regarding patient care delivery
  • Organized, empathetic, and calm under pressure
  • Maintains patient confidentiality at all times
  • Good time‑management skills
