Your Opportunity
We are seeking a Physical Therapist
[PRN] whose primary responsibility is their direct clinical skills, documentation skills, and the compliance to all professional standards.

Key Responsibilities- Responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.
- Responsible for the screening of residents of the community as follows:
New Community residents:
Within 24 hours of admission Long term residents:
Upon receipt of a "significant change" form and/or upon referral from nursing. - Attend all required community meetings and in-services, including Medicare meetings, family meetings, care plans
Minimum
Education Required:
Masters of Science Degree or Bachelor of Science Degree in Physical Therapy. State Licensure.
Experience: As applicable to assignment.
