As a Hospice Certified Home Health Aide (CNA/HHA) with Holistic are Hospice, you will play a vital role in the lives of our patients, working under the guidance of a registered nurse to provide essential services and ensure their comfort and well‑being.

- Provide Personalized Care:
Follow the careful plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregivers, offering compassionate and unwavering support. - Assist with Personal Care:
Support patients with bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity. - Be Dedicated to Customer Service:
Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. - Provide Support for Families:
Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time. - Maintain Accurate Documentation:
Every detail is crucial for high‑quality care, both at the patient’s home and within our agency and regulatory standards. Your attention to detail is vital to our success.
- High School Diploma or G.E.D. required.
- Valid Certified Home Health Aide (HHA) license OR a valid Certified Nursing Assistant (CNA) license required.
- Spanish speaking preferred.
- Minimum of 6 months experience in hospice or skilled nursing facility preferred; hospice experience a plus.
- Possession and maintenance of current CPR certification if state‑mandated.
$20.00 - $24.00 per hour (average pay range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
