Hope Health Hospice & Palliative Care is seeking a Registered Nurse (RN) Case Manager to oversee patient care at home and in facilities across Massachusetts. The role emphasizes advocacy, coordination, and timely documentation, with a 40-hour work week (8am-4:30pm) and rotating weekends.
The RN will assess patients, develop and adjust care plans with physicians, and educate patients and families. MA/RI licensure is preferred; BSN is desirable, with 1–3 years of clinical experience.
