We help older adults and other at-risk individuals enjoy a better quality of care, experience improved health outcomes, and identify chronic conditions that may otherwise go undiagnosed.
- Conduct adult/geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
- Administer and order point-of-care and lab-based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
- Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities.
- Flexibility – Visits are scheduled based on your availability. Assessments are conducted Sunday–Saturday from 8 am–7 pm; you can work during the day, evening, and/or weekend based on your schedule.
- Competitive Pay – Earn income based on the number of assessments completed. Those completing a monthly average of 20–100 visits can earn $1,800–$11,500 per month, based on per assessment rates of $90–$115 depending on state of residence.

- Education and Support – Comprehensive onboarding, assignment of a preceptor, 1:1 feedback, and free access to continuing education.
- Additional Licensure – Support to obtain additional state licensure and credentialing in neighboring states if needed.
- Full-time Opportunities – PRN providers can transition to FT roles with a FT sign-on bonus.
- Master’s Degree or commensurate experience with NP or PA licensure
- Current NP or PA licensure in state of practice with prescription authority or ability to obtain
- Board certified by NCCPA, AANP, ANCC, or AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year experience as NP or PA preferred; new grads encouraged to apply
- Valid state driver’s license and proof of automobile insurance for the state of residence
- Strong computer skills and familiarity with medical record software
- Excellent verbal and written communication skills
