Home Health Aide (HHA) – PATIENT CARE. Develop and manage relationships with our clients in the community to promote OMAMA Home Health Care services and help expand our Medicare home health business in the Worcester Metro Territory. This position requires an individual who is highly motivated, results driven, and able to develop and build strong, lasting relationships.

A paraprofessional trained to perform assigned personal care functions to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN).

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
• Have a sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.

• Understand and adhere to established agency policies and procedures.
• Perform personal care, bath and hands‑on care as assigned.
• Complete appropriate visit records in a timely manner as per agency policy.
• Report changes in the patient’s condition and needs to the RN.
• Perform household services essential to health care in the home as assigned.
• Ambulate and exercise the patient as assigned.
• Perform simple procedures as an extension of therapy or nursing services, e.g. range of motion exercises.
• Assist with medications that are ordinarily self‑administered as assigned.
• Attend in‑service and continuing education programs as scheduled and necessary.
• Attend patient care conferences as scheduled.

Works indoors in agency office and patients’ homes and travels to/from patient homes.

Supervised by:
Director of Clinical Services, Clinical Manager, RNs, PTs, OTs, SLPs.

• Ability to work for an extended period of time while standing and being involved in physical activity.
• Heavy lifting.
• Ability to do extensive bending, lifting, and standing on a regular basis.
