- Provide skilled nursing services in accordance with physician orders and patient care plans at home.
- Observe and assess patients, form nursing diagnoses, counsel patients, and provide health teaching.
- Record health needs, complete start‑of‑care assessments, medication profiles, and develop nursing or home health aide plans of care.
- Evaluate the effects of care, re‑evaluate nursing needs regularly, and revise care plans as necessary.
- Perform nursing treatments and diagnostic procedures requiring specialized skills, assuming full responsibility for each patient.
- Initiate preventative and rehabilitative procedures and modify environment to ensure patient safety and security.
- Counsel patients and their families to meet nursing and related needs.
- Coordinate services with other agencies and health care providers and report changes to physicians.
- Supervise LPNs and home care providers.
- Prepare clinical and progress notes, maintain patient records, and attend in‑service, continuing education, and self‑study programs annually.
- Adhere to applicable regulations, policies, procedures, and maintain privacy of protected health information.
- Perform additional office and field duties as required by the tribe.
- Graduate of an accredited Registered Nurse program.
- Minimum of 2 years RN experience in home health/hospice, skilled nursing, or acute care settings.
- Current negative TB test; current immunizations; successful criminal history and drug screening.
- Current and valid Washington State RN license; valid Washington State driver’s license.
- Current CPR or BLS certification.
- Skills: Effective verbal and written communication, adaptability, patient assessment, monitoring performance, interpreting records, active listening, problem solving, troubleshooting, time management, multitasking, staff supervision.
- Knowledge: Practical nursing techniques, resident care program implementation, medications and treatments, infection control procedures, understanding of physical and mental disabilities.
- Abilities: Follow written and verbal instructions, observe and record data for clinical decisions, evaluate patient conditions, maintain professional attitudes, prepare reports, communicate effectively, maintain confidentiality.
Stand, walk, use hands, reach, lift or move up to 50 pounds; occasionally sit, stoop, kneel, or crouch. Requires close, distance, color, peripheral, depth vision and focus adjustment. Moderately quiet noise level. Possible exposure to chemicals, infectious diseases, and interaction with confused or agitated patients.
