Occupational Therapist Assistant, Home Health

Base pay range: $58,400.00/yr – $80,000.00/yr

Pay per visit/unit: $37.00 – $52.00

• Provide therapy services planned, delegated, and supervised by the qualified Occupational Therapist in accordance with the patient’s Plan of Treatment.
• Assist in the implementation of vocational/education programs and activities established by the registered Occupational Therapist to restore, reinforce, and enhance task performances, diminish or correct pathology, and promote or maintain health and self‑sufficiency.
• Design, adapt, and fabricate equipment and working/living environments to assist and improve function and participation.
• Provide therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist and in accordance with physician orders to improve or restore strength, coordination, range of motion, and function, or teach compensation measures.
• Report information and observations to the Occupational Therapist and/or clinical supervisor, document observed information in patient records, prepare clinical notes, assist with progress reports, and maintain required documentation.
• Instruct patients and family members regarding home programs and adaptive equipment use.
• Participate in care coordination and discharge planning activities, acting as a resource to other health care personnel.
• Design community reintegration activities, coordinating with the interdisciplinary team for physical reconditioning and psychological adjustment.

• Current and unrestricted OTA licensure.
• Minimum of six months occupational therapist assistant experience preferred.
• Home health experience a plus.
• Current CPR certification.
• Good organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.

40 hours per week (full‑time).
