Position Overview

We are seeking a Licensed Nurse (LPN or RN) to join our Physician Support Nurse (PSN) team in the Augusta office. The PSN provides direct clinical and care coordination support to physicians in the care group, ensuring accurate patient information, coordinated follow‑up and streamlined communication with the interdisciplinary team.

Work Schedule & Compensation

Salary‑based full‑time position (8:00 AM–5:00 PM, Monday–Friday).

Key Responsibilities
  • Serve as the dedicated nurse supporting physicians.
  • Prepare and reconcile patient charts with test results, care gaps, consents and documentation.
  • Anticipate physician needs and provide clinical assistance during patient care.
  • Support diagnosis accuracy, documentation standards and quality measure completion.
  • Facilitate communication across the care group to align patient care plans.
  • Perform intake, vital signs and medication reconciliation; document histories in the EMR.
  • Provide patient education on care plans, conditions, medications and self‑management strategies.
  • Monitor ADT alerts for assigned patients and ensure timely physician follow‑up.
  • Coordinate with specialists, hospitals and post‑acute facilities for smooth transitions of care.
  • Schedule follow‑up visits and preventive services to close care gaps.
  • Track and close care gaps, improve documentation and promote preventive services.
  • Manage referrals, labs, imaging follow‑up, prior authorizations and DME orders.
Qualifications
  • Licensed nurse (LPN or RN) license in good standing.
  • Minimum 2 years of clinical experience in ambulatory/primary care, community health or care coordination.
  • Current CPR/BLS certification.
  • Proficiency with EMR/EHR systems and care‑coordination workflows.
  • Strong interpersonal, written and verbal communication skills.
  • Ability to use technology tools for remote patient monitoring, telemedicine and electronic medical records.
  • Team‑player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Flexible with hours, willing to travel between offices and facilities and holds a valid driver’s license.
