Case Manager for Elderly Support Services

Support elderly care at LOFT Community Services as a Case Manager specializing in Behavioural Support in Aurora, ON. Work directly with seniors and their families facing mental health and dementia challenges.

This role focuses on creating and monitoring Behavioural Support Plans tailored to individuals at risk of ALC. You'll be pivotal in training caregivers while working collaboratively with multidisciplinary teams within hospital settings.

Your role contributes significantly to the transitions of clients from hospital to home.

Key Responsibilities:

• Assist clients in developing personalized Behavioural Support Plans

• Serve as an advocate among care teams and families

• Conduct Behavioural Support Assessments accurately and timely

• Provide consultation and training to support teams and caregivers

• Develop crisis plans and manage interventions effectively

Requirements:

• Completion of post-secondary education in Social/Human Services

• Minimum three years of relevant experience with older adults

• Strong skills in crisis intervention and diagnosis

• Familiarity with community resources and behavioural support frameworks

• Valid driver’s license and access to transportation
