Stability Case Manager

Position: Contract Housing Stability Case Manager
Become a Housing Stability Case Manager with Your Support Services Network in Aurora, Ontario. Empower individuals at risk of homelessness through targeted outreach and support initiatives.

This full-time, contract position focuses on delivering essential services in York Region, specifically in homelessness prevention and shelter diversion. You will work closely with at-risk individuals, helping them stabilize their housing situations while navigating access to critical community resources. Utilize your knowledge of mental health needs and trauma-informed care in your role.

Key Responsibilities:

• Executing community outreach to prevent homelessness effectively

• Creating detailed housing stabilization and diversion plans

• Coordinating short-term emergency assistance for clients

• Documenting interactions and tracking progress toward outcomes

• Partnering with multiple organizations for holistic care

Requirements:

• Bachelor’s degree in Social Work or related discipline

• Minimum three years of relevant outreach experience

• Strong skills in advocacy and care coordination

• Excellent interpersonal and de-escalation abilities

• Familiarity with harm reduction practices and housing resources
