Position: Full-Time Case Manager

This full-time role aligns with a Housing First model, where you will work as a housing specialist dedicated to ending homelessness. Key Responsibilities:

• Adhere to Ontario's Intensive Case Management Standards

• Maintain an effective caseload while supporting client needs

• Initiate appropriate referrals to services and agencies

• Establish collaborative partnerships for goal-setting

• Address clients' trauma symptoms effectively

Requirements:

• Degree with 4 years or Diploma with 6 years in mental health

• Current OCSWSSW registration and no qualifying stage

• Bilingual skills in local community languages are assets

• Knowledge of Ontario Mental Health and related Acts

• Valid Canadian Full G driver’s license and insurance required
