Mental Health Advocate at YSSN

YSSN is seeking an experienced mental health Case Manager for a contract role until June 2027, with possible extension. You will be responsible for delivering advocacy services to adults dealing with serious mental illness or addictions. Working collaboratively, you will help clients navigate their recovery process and improve their daily lives while fostering community connections.
Key Responsibilities:
• Assist clients with intensive case management and support
• Advocate for individuals experiencing significant life difficulties
• Facilitate recovery-oriented service interventions
• Perform daily travel within York region for client meetings
• Build relationships with community resources
Requirements:
• Bachelor’s degree in social work or similar discipline
• 3+ years of experience in social services with adults
• Experience in crisis intervention and supportive counseling
• Proficient in navigating housing and homelessness issues
• Police clearance and driver’s license are mandatory
