Position: YSSN Case Manager for Mental Health Services
Make a difference as a Case Manager focused on mental health at Your Support Services Network in Aurora, Ontario. This flexible supports position is a full-time contract role with a commitment to improving client lives.

As a Mental Health Case Manager, you will provide critical case management and advocacy services to individuals aged sixteen and older, assisting those facing mental health challenges and housing instability. Your focus will be on facilitating recovery and enhancing the quality of life for your clients through direct support and comprehensive interventions, with daily travel required within York region.

Key Responsibilities:

• Deliver person-centered case management and advocacy

• Support clients in improving their daily life functioning

• Engage in direct service interventions and clinical support

• Lead community development initiatives for mental health

• Travel daily within York region for client support

Requirements:

• Bachelor’s degree in social work or similar field

• Minimum of 3 years experience in related services

• Background in housing and homelessness advocacy

• Familiarity with relevant laws and cultural sensitivities

• Valid driver's license and access to reliable transport
